Part 1: Speaker: Kellie L. Flood, MD

Title: Function and Mobility:  The Foundation for What Matters, Quality, and Care Transition Planning for Patients of ALL Ages

Function and Mobility: The Foundation for What Matters, Quality, and Care Transition Planning for Patients of ALL Ages is a dynamic, interdisciplinary event designed to highlight the critical role of physical function and mobility in delivering high-quality, person-centered care. Whether working with older adults, individuals with chronic conditions, or patients recovering from acute episodes, healthcare providers must understand how functional ability directly impacts outcomes, quality of life, and successful care transitions.

This session will explore evidence-based strategies and practical tools to assess, preserve, and restore mobility across the lifespan. Participants will engage in conversations around the integration of mobility into discharge planning, interdisciplinary collaboration, and how to use function-focused care to enhance patient satisfaction, reduce readmissions, and support aging in place.

Part2: Speaker: Ethlyn McQueen-Gibson, DNP, MSN, RN-BC

Title: Addressing Risk Factors for ADRD

Addressing Risk Factors for ADRD is an informative and solution-focused session aimed at increasing awareness and action around the modifiable risk factors associated with Alzheimer's Disease and Related Dementias (ADRD). With the growing number of individuals affected by cognitive decline, it is more important than ever to understand the biological, social, and environmental factors that contribute to the development and progression of ADRD.

This session will highlight evidence-based practices and community-based strategies that healthcare professionals, caregivers, and advocates can use to reduce risk, promote brain health, and support early detection. Topics will include the role of chronic disease management, lifestyle interventions, health equity, and culturally competent care in mitigating ADRD risk.
